Differentiation - age spots vs. pigment spots
Age spots are a special form of pigmentation disorder and typically appear from middle age onwards – usually from the age of 40, or earlier in the case of intensive sun exposure. They are usually permanent, well defined, flat and vary in color from light brown to dark brown.
In contrast, there are many other forms of pigment spots that differ in cause, progression and appearance:
- Freckles (ephelides) usually appear in childhood, are genetically determined and become more intense when exposed to sunlight. They often fade again in winter.
- Melasma (chloasma) occurs more frequently in women and is hormonally influenced, e.g. during pregnancy or when taking hormonal contraceptives. These spots are often larger and irregularly shaped.
- Post-inflammatory hyperpigmentation occurs after skin inflammation, such as acne or eczema, and can also leave dark spots.
Prevention of age spots
Even though age spots are a typical side effect of ageing, in many cases their development can be prevented or at least significantly delayed. The most important factor in prevention is consistent protection from UV radiation, as sunlight is one of the main triggers for excessive melanin formation, which can lead to age spots.
The following therefore applies: Use a day cream with a sufficient sun protection factor (at least SPF 30, better SPF 50) every day, even on cloudy days or in winter. It is particularly important to protect sun-exposed areas of skin such as the face, neck, décolleté and hands. If you spend time outdoors, you should also wear protective clothing, a sun hat and sunglasses and avoid the intense midday sun as much as possible.
Skin care: Well moisturized, nourished skin is more resilient and can regenerate better. Products with antioxidant active ingredients – such as vitamin C or E – can help to neutralize free radicals caused by UV radiation or environmental influences that promote skin ageing.
Healthy lifestyle: also has a positive effect on the appearance of the skin. Sufficient sleep, a balanced diet with fresh fruit and vegetables, little alcohol, avoiding nicotine and a good stress balance support skin health in the long term.

Difference between age spots and age warts
Even if age warts are sometimes confused or equated with age spots in everyday life, from a medical point of view they are two different skin changes. The correct technical term for senile warts is seborrhoeic keratosis – also known as senile warts (verrucae seniles).
Both manifestations usually occur from the fifth or sixth decade of life. In contrast to the flat, pigmented lentigines seniles (age spots), age warts are clearly palpable as they are raised and often have a rougher, sometimes scaly surface.
Despite their often conspicuous appearance, age warts, like age spots, are considered completely harmless and are generally no cause for concern from a medical point of view.
Treatment of age spots - using laser and light
Laser treatment
One of the most proven techniques for reducing pigmentation spots in old age is the use of modern laser technology. Among other things, the so-called ruby laser is used, which specifically combats the overproduced skin pigment melanin, a pigment that is responsible for the brownish discoloration of the skin. This method is particularly suitable for clearly visible, well-defined spots.
The treatment usually causes hardly any pain – many describe the sensation as a brief stinging or slight pulling.
How the treatment works
Before starting, the skin is carefully cleansed, then a cooling gel is applied to the affected areas. The laser beam is then aimed precisely at the discolored areas of skin. The resulting heat causes the melanin deposits to disintegrate, which the body then breaks down naturally via the lymphatic system.
Care after the session
After the procedure, the treated skin region is treated with a soothing care cream. In order to optimally support the healing process, the skin should be consistently protected from sunlight in the following days. Slight redness or crust formation are typical side effects – these should not be scraped off under any circumstances, but left to the natural healing process.
What to consider during regeneration
During the recovery phase, make-up, saunas, swimming pools and direct sunlight should be avoided in order to prevent irritation or complications.
What does laser therapy cost?
Prices depend heavily on the extent and severity of the skin changes – i.e. how many spots need to be treated, how deep they are in the skin and how many sessions are required. You can expect to pay between 200 and 800 euros per treatment.
Light treatments
In contrast to laser therapy, which uses a single, highly focused light frequency, IPL technology (Intense Pulsed Light) works with a variety of different light waves. This means that in addition to pigmentation spots, other skin changes such as fine veins, redness or uneven skin structures can also be treated effectively.
The light energy also targets melanin, but is emitted over a larger area and more gently, making it ideal for sensitive skin or larger areas with pigmentation disorders.
Mela Wave
Mela Wave technology is also based on IPL light pulses and has been specially developed to treat age spots, sun spots and freckles. The targeted light waves gently penetrate the skin to break down pigment accumulations and visibly clear the complexion. This treatment is very well tolerated and usually only causes a slight tingling sensation.
Age spot removal without laser treatment
Brightening creams and medical active ingredients
Various creams with bleaching or cell-renewing active ingredients are used to treat age spots without a laser. These include hydroquinone (only available on prescription), tretinoin, kojic acid, arbutin or niacinamide. They inhibit the formation of melanin or promote the exfoliation of pigmented skin cells. When used regularly over several weeks, they can visibly lighten the spots. Consistent UV protection is important, as the skin reacts more sensitively to sunlight.
Chemical peelings
Chemical peels, for example with fruit acids (AHA) or trichloroacetic acid (TCA), stimulate skin renewal and gently remove the top layer of skin. This gradually lightens pigmentation disorders. Depending on the concentration, these peelings can be used at home or performed by a dermatologist. Slight redness or slight peeling of the skin are typical side effects. Here too, sun protection is essential to prevent pigmentation from returning.
