How is photodynamic therapy (PDT) carried out?

Photodynamic therapy (PDT) is carried out in several steps and is relatively simple and gentle. Here is the procedure:

Step 1: Preparing the skin
First, the skin is thoroughly cleansed to remove dirt, oil and other residues. In some cases, it may be necessary to lightly exfoliate the affected skin areas to improve the penetration of the photosensitizer.

Step 2: Applying the photosensitizer
A special, light-sensitive substance, known as a photosensitizer, is applied to the affected areas of skin. This substance can either be smeared directly onto the skin or administered intravenously, depending on the skin problems to be treated. The photosensitizer is absorbed by the affected cells and accumulates there.

Step 3: Exposure time
After the photosensitizer has been applied, it remains on the skin for a certain period of time (usually between 30 minutes and several hours, depending on the type of disease). During this time, the patient should avoid direct sunlight and strong light, as the photosensitizer makes the skin more sensitive to light.

Step 4: Light treatment
After the exposure time, the affected skin area is irradiated with a special light source. This light source emits light at a specific wavelength, which activates the photosensitizer. The activated photosensitizer generates a chemical process in the diseased cells that damages or kills the cells.

Step 5: Aftercare
After the treatment, the skin is cooled to soothe any irritation. Soothing creams or ointments can be applied as required. Slight redness or flaking may occur in the days following the treatment, but this is normal.

Is photodynamic therapy (PDT) painful?

Photodynamic therapy is generally well tolerated. During light irradiation, however, there may be a slight burning, stinging or warm sensation on the skin, similar to a mild sunburn. These sensations vary from person to person, but are usually only temporary. If necessary, the skin can be cooled during treatment to increase comfort. Most patients find PDT significantly more comfortable than invasive procedures.

Are there any side effects of photodynamic therapy (PDT)?

After treatment, you may experience temporary redness, slight swelling, skin flaking or a reaction similar to sunburn. These side effects are normal and are a sign that the skin is reacting to the therapy. In rare cases, crusting or hyperpigmentation may occur, especially if the aftercare instructions are not followed. Serious side effects are extremely rare as PDT is targeted and non-invasive.

How long does healing take after PDT?

The healing time depends on the treatment area, the condition of the skin and the intensity of the therapy. As a rule, the regeneration process takes around 5 to 10 days. During this time, redness, scaling and slight crusting may occur, which gradually disappear. Many patients report a noticeably smoother and fresher skin texture after just a few days. Depending on the treatment goal, a series of applications may be advisable.

What do I have to consider after the therapy?

After PDT, the skin is sensitive to light. It is therefore essential to avoid direct sunlight for at least 48 hours and to use a high sun protection factor (SPF 50+). Sauna, solarium, intensive sports activities or irritating cosmetics should also be avoided for a few days. The skin should be gently cleansed and well moisturized. You will receive an individual aftercare recommendation directly after the treatment.

What does PDT treatment cost?

The cost of photodynamic therapy varies depending on the treatment area, indication (e.g. acne, actinic keratoses, skin rejuvenation) and number of sessions. As a rule, prices range between €150 and €450 per session.
